To me, it has happened using standard dock with fighters in stations/planet rings.
Also on respawn, when you reappear in your last station visited.

If your game crashes, and you're in a relatively small ship, then it's not that. Usually it's either your connection to the server, your game install/mod install, or that the server is having serious lag at the time. Remember, there are on average at least 120 people online at once during "prime time", so that can cause lag as well.

How about the Detroit Munitions base? I can't even undock properly using a starflier, and I know that's happened to lots of players already.

Detroit Munitions is another "depot"-type station. Try to avoid them if you can, otherwise you run the serious risk of getting stuck.
P.S. Usually when I get stuck I'm able to get out by flying above the base then going where I want to go, but it may be only for a couple bases.
