One time, I got 'pulled over' by a liberty police officer. Being new, I'd never seen an "LPI" tag before. I know what "LP" was, but I didn't know there was an "I" on the end. And, seeing the "O" on the end, I automatically connected "O" with "Outcast" and proceeded to scan him, looking for an ID in his cargo. Seeing none, I told him I should blow him up for begin an outcast and for not having an ID. I went on my way undisturbed. Luckily he was kind enough to ignore my deranged ranting. It didn't take long after jumping to realize my idiotic mistake.

About three weeks ago i was in Alpha with my Keeper. Brought a "gift" for the outcasts, a Wildfire i took off a corsair in 99. I swam up to Malta, laviered between the OC dessies and the occasional sabre and dropped the codie between them. The next thing i know a dessie (i think it was a r a something tagged) said something about keeping bbs off malta dock and shot it with a mortair.
Ahh the good times.

During most of 4.84 I was in a Zoner Destroyer exploring the galaxy and I would occassionaly be harrassed by anti-zoner people. My favorite way of losing them was to tell them about the wonderful short cut to NY system through a blackhole or leading them into a guard system that I was (at the time) friendly with.
I guess my biggest fail is all the (house) capital ships I bought in this game: regardless of which house system I would get them, or faction I joined, or tag/IFF I had; somehow I would always ended up chased and cursed by local police / military until it became very uncomfortable to use them. Eventually I would give up and sell them, taking a huge loss in creds, before doing it all over again. :(

For me, it'd have to be when I accused [Lane_Security]Fox of OoRP for having a BHG ID and LPI tag.
I was flying a Rhino at the time, with a Junker ID and Xenos/LR guns. This was early 4.84, but still a fail >.<
Then there was the time I flew my gunboat (cap6) into '58 with Jamez in his 'flea.
It was quite funny to see all forty or so of his deaths-by-weapons-platform. He finally made it when I flew ahead to take the shots.
Oh, and then once he made it, I tried to escape.
I was hunted down by:
- Five Scorpions
- Fifty or so Wraths
- A pissed-off Freelancer Weapons Platform
I -did- escape alive, but with about 2% hull, no bots, and no batts.

Until one day I contacted one of them telling them they are in a restricted area...

Darn, just remembered. Back in the last weeks of 4.84, i was in connecticut with my LR wingclipper. usual madness and being called laggy. One of the perticipants just found out he picked up a mk 8 armour from a zoner or freelancer, cant remember and we started to shoot eachother for the lulz who got it.
I paused me E pressing when the guy was laughing that he had ejected the armour and my nuke hit it...
I never minespammed again.

Ok, someone elses fail, but it is the fail to end all fails.
This huntress undocked from Manhattan and started blasting everything in sight, incuding the starfleas.
Then the LN took it down right after Hoodlem logged on and went in his parking spot at Newark.
It undocks and starts blasting the fleas again.
Seeing this, Hood unleashes his Admin cannons of doom and vapes him.
Huntress F-bombs the navy,
Hood warns him, then comes the FAIL TO END ALL FAILS:
Huntress:"F**k, f**k f**k you admin."
