Oh yeah... I also had a /setmsg fail. I was engaged in the BAFvsKNF event above Leeds orbit, when I told the BAF guys "Under the authority of the Emperor, I command you to leave Kusari space!"
I died a little inside...
EDIT: This was back when KNF hadn't taken any Leeds territoty yet.

Collecting 5 pretty good codes on a 30min run, then getting stuck inside a corsair station while undocking and launching your weapons in frustration.
Because you couldn't simply dock again.
Because you wanted the station turning red on you.
Because you didn't really need those weapons after all.

When i first started in 4.84 i bought a Lane Hacker GB. I decided it would be fun to get some razors for it. So i flew to Omi kappa and bought them from planet gammu. Then i decided it would be fun to attempt to go from there to Alaska and back to Magellan on a busy weekend. I begged 2 corsairs in GB and 1 Order in a cruiser not to kill me in Omi delta. 5mins later they killed me.:$
(worst part was i had no idea i was not supposed to be there...)

Oh I just remembered another fail!
I was flying around West Point on my rogue bomber, shooting up a NPC cruiser patrol. They had knocked my sheilds down for the first time, and as I flew infront of the cruiser forward gun, it shot and insta-killed me...
The expression on my face was something like a mixture of these: O_O, O.o D:

1 Junker/Merc...thing, in a BW Sabre
1 Me, in a Slave Liner.
So I decide to help the LPI officer, as his prison supplies me with slaves, right?
Also the Junker/Merc in a Sabre seemed like a good slave.
Flying forward, shooting backwards.
And then...oh no what is this?!
Zone 21 MINEFIELD! Right in my face.
Yeah, then the guy shoots me a few times as I'm backing out of a bathing of explosives, and I 'splode.
I thought it was funny...he probably thinks I'm stupid.

I remember when I first started. I had bounty hunter chara in a Havoc bomber (don`t ask) equipped with only a nova torp launcher. I went to Alpha and attacked an outcast cruiser, figuring "I have a bomber, so I should win against a cap ship". I didn`t win. a lesson learned in 4 seconds flat
anoter one from my starting days: I was repping another hunter chara. so I went to sheffield and bribed to fix my rep. back then, I thought bribes were infinte (how dumb can you be?) so after 2 bribes I was still trying to get bribes to get my rep full green. I was "select new character-ing" for over 30 minutes wondering why that ruddy bribe wouldn`t show.:$
