In the navy, the title Captain is given to the commander of a capital class ship, so regardless of his real rank (probably sub-Admiral or something like that) he would be referred to as a Captain while on the ship. I think.
Naval history and etiquette has never been a particular interest of mine.
However, what really irks me is if you attempt to RP chain of command effectively, people tend to get confused. For example, I was playing a 2nd Lieutenant in a Templar, and I was flying alongside a GB and a BAF pilot who was a captain. I referred to the GB as 'sir' and the BAF pilot as Ma'am as a junior officer would to a senior officer.
The GB captain then asks why I was calling the captain Ma'am, and if 'I was her servant or something'. I then had to enlighten him to the fact he was actually an officer too...
If only people would research a character's background...
