Skyrim's main issue for me was how easy it was to 'accidentally' exploit imbalanced mechanics. Upgrading armour and weapons (with potions and enchanted crafting gear, even) led to the game becoming easy peasy. You didn't even need to try hard to create a dagger you could one-hit-kill your average dragon with.
Having to deliberately hold back from using overly powerful skills or combinations of skills, to me, means the game is poorly designed in that aspect. I should be challenged to find good builds and playstyles, not be forced to avoid them to find a challenge.
