They tend to need to be forced down on a table, and stuck with needles.
So, are you suggesting that some factions need to be forced, and others are fine? Or that no one needs ever to be looked at?
You cannot make it optional, because then the sick people, to continue your metaphor, stay sick.
The factions which 'need' it will probably not come, while those who don't probably will, which defeats the point. And to spare us deciding which factions are sick, we'll say that they're all sick, and expect everyone to get vaccinated.
