Captchas are less effective than questions. If you land question, eventually script will try to BF its way trough it via picking random phrases from other pages. If thats not effective, guy from Ukraine, Russ, Bangladesh or other India will sit to it and will break it himself. This takes while longer than captchas where you basically have trained AI's breaking most common captchas themselves and sweatshops filled with people that do nothing else but answer those for few hours a day, for 80 cents per 100 solves. Because those captchas are offered by tird party service, systems were developed that can just display the captcha that needs to be solved on guy's screen and when he/she solves it, pop another one.

Myself, I would look into MediaWiki plugins that score likeness of edits being spam based on their contents and user's trust levels... or just stick to changing security question frequently and giving latest changes a look on regular basis.
