Do rewards stack or I have to spend X points for Y reward? E.G. when I get 12 points, do I only get engines or also custom title and cap code?

Your points accumulate, so no. If you get 12 points total, you can only have either a cap code and a title or an engine.

Also what if e.g. my ship encounters more people at once - are there restrictions there? I.e. I park a ship somewhere, people go to it, RP and 5 take my rum - what are the rules there if they aren't grouped, come at different times, what if they are?

If you encounter multiple people at once (you being alone and encountering a group of 4 traders that might be interested in chatting with you), there are no restrictions. However, you may not have people approach you. It should be the other way around. I will smell foul play if I see it.
