Greetings once again, friends. Indeed, though once the procedure was explained in detail to many of my charges, they elected not to undergo this endeavor, several of those placed under my care have eagerly applied for an opportunity. I Annete Brown, as an inmate and/or crew member of the Zoner prison liner Sacra Profani hereby give consent to undergo medical procedures at the hands of Aperture Science, whereby I will be genetically altered to the degree to be determined in my application. By signing this contract I confirm that I am not under any compulsion by any other party, and make this decision whilst being of relatively sound mind and body.

Captain's Agreement

I Solaris Solani, Captain of the Zoner prison liner Sacra Profani confirm that any and all inmates and/or crew members of this ship who apply for this procedure have done so by their own free will.
Upon completion of the procedure, I confirm that the parties in question will be granted a full pardon for any and all crimes committed against the Zoner people, and thus be released from bonds. In return the Zoners will receive full disclosure of all medical and genetic data gained from this experiment.
