The safest rule is: If you have to question it, Don't say it.

OORP Insults are quite idiotic to me. If you really have to insult someone, specifically the someone that just blasted you into a million tiny pieces in a game, why bother typing it all out in chat to them rather than just yelling that the computer?

In-RP Insults are insane. The key is not to use anything that might be remotely considered a "bad word" but rather string together normal words (or made up gibberish words "Mekfad") in order to make an "insult" that really drives it home. "You lousy little noodle slurping, slanty eyed, 'san'-saying, Samurai Decendant!"

Like so, I've just attacked a Kusarian's Ancestors, Traditions, Appearance, and way of speaking all in one string of totally clean words.

The imagination can provide some good insults.
And note that a lot of "swear words" nowadays were just insults like that which got rather popular and then were used exclusively for the negative "swear" meaning, due to their popularity as it.

Tbh, I don't see why swearing is disallowed as long as it's not used for flaming. When people say "frak" for example, it's pretty much obvious what they actually want to say.

There, I said it.

Other than that, made up insults (as long as they are not relevant to anything already existing) add to the lulz of an encounter, the way I see it. They are nothing but a source of humor as they are definitely not insulting enough to be taken seriously.

My guy cusses like a fighter pilot. Because he's a fighter pilot.

However I've kept it to "Damn" "Goddamn" "Ass" and "Bastard". The F, B, D, P, and C words never cross my lips. The S word, if ever invoked, is just "Oh Sh-"

Needless to say the worst I can call someone is a "Goddamned Bastard", and usually the swears are just exclamatory. "Aw Hell, Goddamnit, I lost my damned target.." It's little different than the RM going "VERDAMMT!" but with more fighter pilot undertones and more English.

Note that, in reality, this is cleaner than your average eight year old. Play Xbox Live for more than an hour and you'll get my point. If you still don't get it, ride a bus home with elementary students every day, and sit near the front where the youngest kids gather. If you still don't get it, jump off a bridge and hopefully a little kid walking around nearby will shout a given expletive.
